ABOUT SCLERODERMA

Scleroderma or Systemic Sclerosis is a rheumatic and auto immune disorder wherein the skin and connective tissues get tightened and hardened. Scleroderma is made from two words “Sclero” which means hard and “Derma” means skin. The problem gradually gets worst and so Scleroderma is a long term progressive disease. There could be blood vessel problems, inflammation and pain in internal organs and the digestive tract due to Scleroderma.

Scleroderma leads to overproduction of collagen resulting in thickening and scarring of tissues. The problem is more common in women of mid age. Genes play a role in passing on the disorder. It is not a contagious disorder. It is not cancerous, malignant or infectious.

Scleroderma can be considered as localized or widespread (when internal organs are affected). It can also be classified as limited and diffused depending on the extent of skin affected.

According to Ayurveda, Scleroderma is a form of stubborn skin disease which is due to simultaneous imbalance of all the three energies of Vata, Pitta and Kapha. These disrupt the tvak or skin, ambu or lymph /rakta dhatu and mamsa dhatu.

The imbalance of Vata energy leads to roughness and dryness of the skin. It may also lead to hardness and thickening of the skin making it look more brown or red.

The imbalance of Pita energy leads to burning sensation and cause the skin to shed. There could be some discharge from the skin too.

The imbalance of Kapha will cause white discoloration and elevation of skin. The skin will be cold to the touch.

SYMPTOMS OF SCLERODERMA

Inflammation, redness, swelling, tenderness, itching and pain in the affected skin is the most common symptom of Scleroderma. There could be red spots on the face and hands.

Skin may appear to be shiny as it is tight. There could be hardening and tightening of patches of skin leading to restricted movement of the affected area eg in case of facial skin, the tightening makes the mouth look smaller and narrower making it even hard to brush the teeth.

One may experience emotional distress and sensitivity to cold temperatures. There could be numbness, pain and color change in the fingers and toes.

Scleroderma affects the esophagus thus the patient experiences acid reflux or heart burn along with other digestive problems like difficulty in swallowing. Some may experience constipation and diarrhea/on off.

Function of heart, lungs, kidneys are also affected in Scleroderma. One may feel weak and tired due to reduced ability to breath. It could cause high blood pressure too. Abnormal heartbeats can be experienced by the patients. Patient has headache is a common symptom.

Symptoms may be triggered by exposure to certain pesticides, epoxy resins and solvents in some cases.

Scleroderma often affects the sexual function of the patient. In men, it leads to erectile dysfunction while in women, it might decrease lubrication and constrict the vaginal opening.

DIAGNOSES OF SCLERODERMA

Consultation with a rheumatologist or arthritis specialist and a dermatologist or a skin specialist is required who conduct blood tests and other specialized tests to check which organs are affected by the disorder. Biopsy is done to examine the tissue for abnormalities.

Tests like CAT scan to examine lungs, EKG and echocardiograms for the heart along with heart catheterization are done , lung function testing or pulmonary function test , upper and lower gastrointestinal tests etc are performed.
