About Fish Odor Syndrome (Trimethylaminuria)

Trimethylaminuria (TMAU) is a rare genetic metabolic disorder that occurs when the body of a person is unable to break down trimethylaminuria which is a nitrogen containing compound. Fish Odor Syndrome is characterized by an offensive body odor that smells like rot fish, feaces or garbage. This is also known by other names like State fish syndrome, TMAU and TMAuria. This odor is due to the excessive secretion of a protein called tri-ethylamine (TMA) in the urine, sweat, and breath and reproductive fluids, producing a strong fish-like odor which varies in intensity over time.

TMAU, also known as fish malodor syndrome which is caused by defects in a protein called flavin-containing monooxygenase 3 (FMO3). The FMO3 protein’s functions mainly to break down nitrogen-containing compounds. This enzyme produced by the liver.

TMA is consumed by diet and is it is broken down through a process called N-oxygenation.

The main symptom of fish odor is mostly present from birth. Prevalence of Fish Odor Syndrome is more likely to affect women than men, because female sex hormones like progesterone and estrogen aggravates the odor-related symptoms with this disorder. Trimethylaminuria (Fish Odor Syndrome) is a rare genetic condition with unknown causes.

Symptoms of Fish Odor Syndrome

  • Symptoms of trimethylaminuria (TMAU) include offensive body odor, high blood pressure and increased heartbeat.
  • People who suffers with Fish odor syndrome can sometimes be seen in premature infants as well as in some women at the time of menstruation. In some people it is characterized by constant fishy odor.
  • Factors such as sweating, stress, excessive exercise may also worsen the odor.

Causes of Fish Odor Syndrome

Fish Odor Syndrome is caused by mutations in an enzyme called flavin-containing monooxygenase 3 (FMO3). This FMO3 gene responsible for the production of the enzyme by the liver.

Therefore the faulty FMO3 gene is the main cause of Fish Odor Syndrome. There are some other causes:

  • Poor hygiene.
  • Liver disease or Kidney disease with underactive FMO3 enzyme.
  • Excess proteins in the diet.
  • Increase in bacteria in GI tract which lead to trimethylamine production.
  • Gingivitis
  • Cases of blood-borne halitosis.
  • Heredity: If the parents have trimethylaminuria, then children will be carriers of the faulty gene. In addition, Stress and diet play important role in the existence of Fish Odor Syndrome.
